Meyer/Lyden 2001 modified NIH Stroke Scale (mNIHSS): eleven caller-assigned NINDS-style integers. Drops LOC, facial palsy, limb ataxia, and dysarthria; collapses sensory to 0 = normal or 1 = abnormal; keeps left/right motor arm and leg (each 0-4). Sum 0-31. magent does not examine the patient. No total-score severity bands in Meyer 2001; later website cutoffs are not applied. Not Brott 1989 NIHSS, not a tPA order, not a diagnosis, and not a medical device.
