Liu 2004 respiratory clinical score: four caller-assigned 0-3 domains — respiratory_rate (age-banded Table 1 ordinal, not raw breaths/min), retractions, dyspnea, and auscultation. magent sums only (max 12) and does not count breaths or auscultate.
