Job Description A bit about this role: We want to help patients navigate the healthcare system in a better and safer way, feel their best, and better manage their chronic diseases. Our transitions of care program is critical to achieving these goals, and TOC Coordinators will play a key role in supporting these programs. As the supervisor for the TOC Coordinators, you'll lead a team of TOC Coordinators in serving as advocates for these patients, supporting the TOC clinical team.
